You will find the Insurance Customer Service phone number on your card. Please check your insurance coverage carefully by asking the following questions:
- Do I have mental health insurance benefits or outpatient visits?
- Is this provider covered? If so, what is my co-pay per visit?
- What are my out of network provider mental health benefits?
- What is my deductible and has it been met?
- How many psychotherapy sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician.
- IN-Network Provider benefits are available for: Community Health Plan and Wellness Corporation. This includes employees of CHOMP, SVMH, Carmel Manor, and MIIS. You are responsible for your co-pay at the time of the visit.
- You may be covered to see me if you have an insurance plan where I am an OUT-of -Network provider. I can help you to determine your coverage. You will be responsible for full payment at the time of visit and will be given a receipt that you may submit to your insurance for reimbursement.
